Urea constitutes approximately 45% to 50% of total non-protein nitrogen (NPN) in blood. It is synthesized exclusively in the liver via the urea cycle (ornithine cycle) as the primary excretory vehicle for surplus metabolic nitrogen.
Jaffe's classic kinetic or end-point reaction involves the condensation of creatinine with picric acid in an alkaline medium (pH > 12), forming a characteristic red-orange Janovski tautomer complex measured photometrically at 500 to 520 nm.
Serum creatinine concentration is proportional to individual skeletal muscle mass. The normal adult reference range is 0.7 to 1.3 mg/dL (62 to 115 ┬╡mol/L) for adult males and 0.5 to 1.1 mg/dL for adult females.
Non-creatinine chromogens including acetoacetate, acetone, cephalosporins, pyruvate, and glucose react with alkaline picrate, causing false-positive elevations in traditional end-point Jaffe methods. Kinetic rate Jaffe assays minimize this interference.
The normal biological reference interval for Blood Urea Nitrogen (BUN) is 7 to 20 mg/dL (2.5 to 7.1 mmol/L urea). To convert BUN to total serum urea, BUN is multiplied by a molecular factor of 2.14.
The enzyme urease specifically hydrolyzes urea into ammonium (NH4+) and carbonate/bicarbonate ions. In the Berthelot reaction, ammonium ions react with hypochlorite and phenol in the presence of sodium nitroprusside catalyst to form a blue indophenol dye.
